The Woodlands Hospital Pharmacy Residency
CHI St. Luke’s Health–The Woodlands Hospital, part of CHI St. Luke’s Health System, is located in The Woodlands, Texas. The Woodlands Hospital offers clinical services including cardiovascular services, diagnostic imaging, women’s services, neurosciences, pediatric care and surgery, pathology, pulmonary services, and sleep disorders.
Clinical affiliations include The University of Texas MD Anderson Cancer Center’s Radiation Treatment Center and Texas Children’s Hospital. Innovative programs and initiatives involve outcomes management, personalized care, healing environment, spiritual support, and community outreach.
The integrated pharmacy practice model at CHI St. Luke’s Health–The Woodlands Hospital aims to optimize safety and efficiency in the medication use process. Our expert staff of pharmacists and technicians partner with physicians and nurses to provide optimal care to our patients.
The clinical pharmacist staff oversees and advises the clinical care teams on all steps of the medication use process and provides direct patient care throughout the hospital. The certified pharmacy technicians prepare sterile products, manage first doses and unit-based cabinet refills, barcode and package unit dose medications, check other technicians’ work, and provide support throughout the department. Our transition of care program engages certified pharmacy technicians to collect patients’ medication histories for clinical pharmacists to review and facilitate medication reconciliation process.
The Woodlands Hospital adopts advanced technology to advance patient safety, including barcode medication administration, computerized physician order entry, and an integrated electronic health record. Pharmacists have easy access to patients’ electronic medication records and utilize clinical reports to efficiently monitor medication therapies. Medication distribution is provided from a central pharmacy, unit-based dispensing cabinets, and USP 797 compliant cleanroom.
The post graduate year one (PGY1) pharmacy residency program at The Woodlands Hospital has pre-candidate accreditation status by the American Society of Health-System Pharmacists (ASHP). The program offers a series of rotations designed to provide comprehensive core-learning of diseases and drug therapy issues pertinent to patient care.
Core rotations include critical care, emergency medicine, infectious diseases, informatics, admission medication reconciliation, discharge transition of care, and pharmacy administration.
Elective rotations may include select rotations at CHI St. Luke’s Health–Baylor St. Luke’s Medical Center, such as medical cardiology, cardiothoracic surgery, cardiothoracic transplant, liver transplant/hepatology, renal transplant/nephrology, hematology/oncology, and others.
The residency includes longitudinal experiences in medication safety, two projects, practice leadership, and concentrated experiences in professional development and medical emergencies.
Residents have the opportunity to engage in teaching and preceptorship of students during selected rotations. A certificate in academia is offered through the University of Houston College of Pharmacy.
