RESIDENCY REQUIREMENTS:

  • Texas pharmacist licensure must be in good standing by the 6th week of residency
  • Staff every third weekend
  • Staff at least one major and two minor holidays
  • Start Residency no later than July 15th
  • One major and one minor project
  • Medication use evaluation
  • Manuscripts on projects and medication use evaluation
  • Code Blue participation
  • On-call responsibility
  • Case Presentations and Seminars

You must apply online through the ASHP PhORCAS program (www.ashp.org/phorcas) (application documents are not to be mailed).

The following documents are required to be submitted as part of a complete application for the PGY2 Residency in Critical Care Pharmacy Practice.  The deadline for application is January 6th at 11:59 PM

  1. Cover Letter -- please provide a cover letter for your application materials which addresses:  a) your short and long term career goals, b) your thoughts on how a critical care residency will help you to achieve these goals, c) why you feel that a specialty residency is now the best career path for you, and d) any information about yourself and your candidacy which may not be apparent on other documents.
  2. A copy of your curriculum vitae (CV). 
  3. Most recent College of Pharmacy and/or Graduate School transcript(s). This is waived if you are more than five years post-graduation.  The transcript can be an unofficial copy.
  4. Letters of Recommendation:  Three (3) letters of recommendation are requested.  If you are within three years post-graduation, one should be from a faculty member, specifically from a clinical rotation preceptor, one from your PGY1 pharmacy practice residency program director, and one from an employer or from someone outside the profession. If you are more than three years post-graduation, one should be from an employer/supervisor, one from your PGY1 pharmacy practice residency program director, and one from someone outside your employment.  A faculty person can still be used if an ongoing relationship has been maintained. No set format is required for the letters; they need to address your potential for success in the residency program.
  5. A passport size photo (pdf file)

An on-site interview is considered very important and is required for a complete application.  All applications will be screened by our Residency Advisory Council, and all applicants chosen for interview will be contacted by the middle of January.   The on-site interview is a full day and will include a fifteen minute presentation on a disease state management topic of your choice to a group of pharmacists.  You will interview with two groups of Clinical Coordinators and Managers as well as Residents.  The last date for interviews is normally one week before the ASHP match list submission date.   Financial assistance for the on-site interview is not available, but we will work with you in any way we can to reduce expenses.
