One2One Diabetes Care is an on-going program that provides you with the knowledge and skills to modify and control your diabetes. This program is provided in addition to your regular appointments with your physician to monitor your diabetes.
As part of One2One Diabetes Care, you will receive guidance from a Certified Diabetes Educator® (CDE)—an expert in diabetes prevention and management—to understand and manage your condition.
In a one-on-one or small group session, a CDE will come to your physician’s office on a weekly basis to empower you to have the best quality of life.
Diabetes affects many different areas of your life. To manage diabetes successfully, you must learn to set goals and commit to a lifestyle by eating nutritious foods, taking medication, and exercising regularly. To help you achieve these goals, ask your physician to refer you to Baylor St. Luke’s Medical Center One2One Diabetes Care.
