Are you a Physician interested in providing One2One Diabetes Care for your patients?

CHI St. Luke’s Health–Baylor St. Luke’s Medical Center is dedicated to addressing the diabetes epidemic in community patient populations. To achieve this, Baylor St. Luke’s invites physicians to implement into their practices One2One Diabetes Care, a supplemental program to help patients self-manage their diabetes and maintain a healthy lifestyle.

On a weekly basis, a Certified Diabetes Educator® (CDE)—an expert in diabetes prevention and management—is integrated into the physician’s clinical operation. The CDE works compliantly within each physician’s office to provide One2One Diabetes Care.

How It Works

Once a physician incorporates One2One Diabetes Care into their practice:

  • The CDE will perform a baseline assessment, which is a comprehensive set of questions related to diabetes and each patient’s health.
  • Based on the assessment, the CDE develops a customized treatment plan tailored to each patient’s needs.
  • Patients will meet weekly with the CDE to receive:
    • One2One mentoring
    • Nutrition, wellness, and weight loss education
    • Diabetes self-management tools
    • Methods to set and reach health goals
    • Support from CDE and peers
  • The CDE will collaborate with each physician to refer appropriately via the ADA guidelines to specialists, such as endocrinologists, podiatrists, and cardiologists.

Throughout the program, the CDE communicates with the physician and collaborates with medical teams to ensure the patient’s treatment plan is in accordance with their specific medical needs.

Once patients complete the program, they will be better equipped to self-manage and keep their diabetes under control as well as be motivated to maintain a healthier lifestyle.
